The Bank Comparison sheet should accompany all equipment quotes. It explains why the customer should use the leasing program rather than using their on-hand or bank-extended working capital.

The Credit Application starts the application process. On this website, you can complete the application online and either submit it online or print & fax it. Also, you may download a blank form below to be completed offline. Please be sure to fill out the form completely and sign it. Once finished, just fax it back to the number on the application with the job quote. It's as simple as that!! Keep in mind that if the project exceeds $75,000 we may request the last two year-end financial statements plus any interim statements. If you do not have audited or accountant-reviewed statements, please send in your last two year Federal tax returns.

If you are a dealer or contractor and this is your first leasing transaction with our leasing office you should download and complete the Contractor Profile Form to submit with the Credit Application.

If you are a religious or Not-For-Profit organization (501c3), you should also download the Not-For-Profit Questionnaire. Fill it out and then fax with the Credit Application to start the application process.
